A five-point scale may look objective, but it creates inconsistent results when managers and raters attach different meanings to each number.
Define every rating
Describe the frequency, quality or impact represented by each point. Avoid labels such as good and excellent without behavioural guidance.
Include not observed
People should not guess when they lack enough evidence. A genuine not-observed option protects data quality.
